A new category of software failures, termed 'quiet failures,' is emerging in AI systems where monitoring dashboards show normal operations, yet the system's decisions gradually become incorrect. Unlike traditional failures where a system crashes or alerts are triggered, these issues involve subtle drifts in behavior due to complex coordination and feedback loops in autonomous systems. Traditional observability metrics like uptime and error rates are insufficient for detecting these problems, as correctness depends on sequential interactions and emergent behavior over time. Addressing this requires a shift towards behavioral reliability and supervisory control systems that actively monitor and steer system actions.
